RHS Health Center
Heartland Health Center at Roosevelt provides primary care, dental, and mental health services--all at no cost to Roosevelt students.
Call (773) 866-0818 or stop by the health center to schedule your school physical, immunizations or dental appointment. The health center also provides preventive health care education, treatment of acute health problems, initial emergency treatment of injuries and illness, referral and linkage to other appropriate resources.

Referral to mental health services-- students, parents, and staff can refer students for mental health services by printing out this form, completing the top portion and dropping it off at the health center or placing in the health center mailbox in the main office.
